What is Reality Heaven/Hell/Ghost as per Buddha and Bhagawad Gita?

When Buddha says Enlightenment and Nirvana is only way to end suffering then how can hell or heaven as separate place exist. When Bhagwad Gita teach good quality(forgiveness, Non-violence, selflessness,etc) is attribute of Divine being(chapter 16) then how can Indra who fight for his kingship be divine. When moksha and enlightening is way to end suffering as per Gita then how come hell or heaven as separate place exist. Hell or Heaven or Ghost etc is mindset not separate entity.
